DPN Therapeutic Case Manager: 

  • The Therapeutic Case Manager position offers the opportunity to work within a fun, supportive team environment.  This position would be a great fit for a clinician interested in gaining and continuing their experience working with children, youth, and families within a community mental health agency, in the community, and school systems by providing therapy and case management services, providing outreach, and utilizing skills that require one to be detail-oriented.  Family Advocate/Home Visitor:

  • Under the supervision of the Center Director and support Program Management, the Family Advocate-Home Visitor will provide prenatal, child development, health and family support services to up to 22 pregnant women, infants, toddlers and/or preschoolers from birth to five years of age.  The position includes conducting up to 6-weekly home visits and working as a member of a teaching team at one of our licensed child care centers located in the City of Alameda. For full job description click HERE.

AFS is a non-profit agency that provides comprehensive supportive services to youth, families and children in the City and County of Alameda.
